Contract Duration: 6 months (with possibility of extension/conversion) Summary: The Care Coordinator supports members by completing health and psychosocial assessments, developing initial care plans, and coordinating services with an inter-disciplinary care team (ICT). The role centers on telephonic/electronic outreach and ensuring members receive timely, culturally competent support. Key Responsibilities: Conduct health and psychosocial assessments. Develop and document individualized care plans with the ICT. Identify barriers to care and implement interventions. Educate members/caregivers on resources and benefits. Coordinate with facility and community-based care managers to ensure smooth transitions and avoid duplication of services. Maintain accurate, timely documentation and share process improvement ideas. Requirements: Diploma, Associate’s, or Bachelor’s in healthcare-related field or equivalent experience. 3+ years in care coordination, case management, discharge planning, or related healthcare role.
